You cannot apply for Social Security disability benefits without completing the prescribed application and Disability Report.   If you write a letter to Social Security about wanting to file an application, they will consider it an "intent to file a claim" and contact you for an application on their prescribed forms.   If you fail to file a formal application, they will issue you what is called a Close-Out letter and your file will then be closed
