You can start from seed (trees will bear seed in late summer/early fall) in planting medium in small pot, or usually you can find seedlings already growing underneath or close to a mature tree if there is an area with bare soil available.

Sure, but prune it at the joints, as close as possible to trunk or branch. The blooms are only on new growth, shape the plant, don't cut it like a hedge. Crepe myrtles came from a cutting from France, 1920, if you take cuttings before they leaf out, stick them in dirt, they root very easily ...

Although it is not usually recommended the best way to get a tree to come back is to use a commercial strength fertilizer. I own a tree care company that treats trees and I have had great success bringing back dead trees. I hope this helps. Bryan
