Your best bet would be to contact their HR dept (should be able to find the contact on their web site).  If you still have contact with any of your co-workers, they might be able to give you the name and number of the current payroll supervisor.  If all else fails, you can contact the IRS and they can send you a transcript of what they have on file.  An exact copy of the W-2 is not available from the IRS, since it is filed with the Social Security Administration.
