Why not learn from the best:

When teaching any swimming stroke break it down into its essentials components, and have each swimmer practice the components seperately.  Practice just the kicking motion, then the arm motion and then the breathing techniques.

Look straight down to the bottom of the pool, not in front of you. If you push your head and chest down in the water, the buoyancy of the lungs will make your hips and legs come up. Then you don't need to kick hard anymore, you will need less oxygen, and breathing will be easier.
