Survey: "How do You RE-Order Product?"

I am researching consumer habits when RE-Ordering products that were previously purchased on-line.

Products like toilet paper, diapers, household products, health and beauty items, grocery items, and pet products fall into this category.

I would be grateful for your time in completing my survey:

Any other comments on how the re-ordering of consumables (like the above) could be done better, can be entered as answers.

The survey should take no more than 5-7 minutes to complete and is anonymous.
