Scar tissues are fibrous tissues which restricts the joint movement following knee replacement surgery. When scar tissue forms under the patella it restricts the patellar movement and thus flexion gets restricted and when therapist tries to forcefully flex the knee it becomes extremely painful.

Hope it helps.
