It could be that a baby was left on the church steps or something.  If the mother abandoned her child, the parents would be unknown.

There are now designated places where an unwanted baby can be dropped off.  Police and fire stations and hospitals may be designated safe havens.  The baby should be brought inside and given to a person, not left outside.

Well,,, I'm gust going to take a wild guess here, but I'm thinking it means the identity of the parents were unknown when the kid got left at the orphange.

Do they even have orphanges anymore?

Hay. You think I'm crazy? You should hear the voices in my head. Those people have issues.

I would imagine it means the same thing with a step child as it would with anyone. And it's not a term, which is two or more words spoken as a colloquial idiom.
