Can a person with lewy body dementia understand your conversation with them although they cannot talk to you???

I think you have to assume that at least some of the time that they can.  My wife has had LBD for many years.  She has not been able to have a regular conversation for about a year.  She can show emotion by smiling, laughing or sometimes, crying.  Sometimes she will say a series of words that do not make sense.  Sometimes she just stares into space and does not respond when spoken to.  Once in a while, she responds with a word or phrase that seems appropriate.  When people smile at her, she often responds with a smile.  There are times of the day when she is more responsive, I guess like all of us.  Sometimes old friends call on the phone or visit and I am convinced (or want to believe) that she recognizes their voice.  Often she will make noises in response to their voice. I try to be as gentle and kind as possible when I talk with her.  I tell her she is beautiful and that I love her.  I want to think she understands at some level.
