Regardless of a person's age, the best practices regarding interviewing remain constant.  The main elements include:

Doing your research and homework ahead of time.

Finding out something about the person or people you will be interviewing with.

Know what they are looking for specifically.

Ensure that your skill set is a great fit.

Have a targeted resume that showcases your greatest strengths and accomplishments.

Prepare your 2-minute pitch.

Use a consultative approach during the interview.  You need to find out what their challenges are and what you intend to do to solve them. 

Try to find out their timeframe for a decision.  How many people are they interviewing?  Where are they in the process?  How do they see you stacking up against the competition?  Would they have any concerns about bringing someone like you onboard?  You need to try to find these things out!  If you do uncover an objection, don't address it right there, that would seem silly.  Instead demonstrate through effective follow up how their concerns will not be an issue.

Take good notes during your interview so you will be prepared for follow up.

Use your follow up to knock the ball out of the park. 

Keep control of the process rather than taking a passive approach.

Thanks for the great response Jennifer.  All good advice and I am always prepared for each interview.

My resume and follow-up phone interviews go beautifully.  It's only when I get to the in-person interview where things look and sound like they are going well, that I fall out of the candidacy.  Until that point, many potential employer reps indicate they are very excited about the possibility of bringing me on or feel fortunate to have found someone with my skill/experience match-up.  Then I get a "thank you for applying" note or "we will be pursuing other candidates" response, only to see the same ad reposted asking for someone with one or two years of experience, which was not listed before.  It's not salary, as we both discuss frankly before any interviews that I'm flexible, nor is it my references because I don't get to the point of submitting any.

I guess it's a fact of life to be accepted but is there a way to prepare a potential interviewer for the fact that I'm not in my 20's or 30s?

I don't think there should be a difference.
