Incoming mail going to spam folder when spam controls are turned off

Why are emails from people in my address book going to my spam folder when I don't have spam controls turned on? Also, why do some emails never make it to me at all? I know this because I forward from other accounts.

The Spam controls are in menu Mail, Block Unwanted Mail. You can raise the level of filtering from Low to Medium or even High, but more legitimate mail will go to the Spam folder. I have mine on Medium. You can add certain words to the Content filter. For example, you can have Viagra, pharmacy ...
