"Health Insurance Research Fee Is this per month, per year, per what?

According to the revision, the government will begin charging a fee in order to conduct pharmaceutical research that will help find out which drugs, medical procedures, tests and treatments are most effective.

I know what the fee is for.  What I've been asking is will that fee be once a month, once a year, or what?  Since it will double the second year and increase with inflation after that, I assume it is NOT a one-time thing.  So far, I have asked the same question regarding the FREQUENCY of the fee and of two responses, that question has NOT been answered.......
