How do I get inside my faucet to fix it?

When I turn my cold water faucet nothing happens -- no water.  I can feel that the grip on the ridged post inside the faucet is loose, but I don't know how to open the outer decorative part of the faucet to see what's wrong inside.

A lot of times, the little cap on top that says H or C pops off and the screw holding the handle to the stem is under it.  Sometimes there is an allen screw or set screw of some kind that hold the handle to the stem.
