First; I am letting you know up front, I am A certified Special Education Teacher.  The problems are both POLITICAL causing policies!
First Politically; parents and PTAs demanded that our students were not scoring like those from the Asian countries and the former SSR/Eastern Block, which put lower testing people or groups they saw as less inelegant have been put in lower level jobs, (jobs for those who were considered illiterate/retarded...) Thus the federal Government got pressured by parents that their children were not learning, when in fact the students were not completing assignments on time or just not turning them in.
So A group of politicos got together and created the "No Child Left Behind Act" ; I still Do NOT believe any teachers were involved in creating it.
"No Child Left Behind" grades schools who in turn grade teachers based on how ALL students do on a government test The first time the test were given Most schools and teachers FAILED, so under orders from the higher ups teachers were forced to teach the test. This means at least 90% of what is taught is the test. Then they had to make sure certain students were not tested. I know nearly Every school in every State will deny this but I know Teachers in more than one state and the majority feel and say the same thing!
