I have seen this question each time I answer medical questions and I always include ask your physician! Not applicable for a dog -- ask your veterinarian  would be appropriate.

Levaquin is a fluoroquinolone and the fluroquinolones are used to manage multiple canine diseases. Yes, you can give a dog Levaquin but you need to know the dosage and that is determined by weight.

IF you have not consulted a veterinarian for your dog, then I suggest that this is important . . . most do not charge for a phone consult when they have treated a pet in the past.

A pet is a responsibility and proper health care is a part of that responsibility!
