An adrenaline surge caused by fright, anger, anything that triggers the adrenal gland. 

Or it could be the 14 Viagra you took.

A large dose of speed.

smoking can cause an increase, although i wouldnt consider it a significant spike. Pain will also cause an increase along with stress, and cardiac related issues.
